All yonko have been seen to go all out. All except one. One who is repeatedly been described as inferior to Mihawk. Mihawk who hasn't shown 2% so far.

depends what the title is about
if its some niche skill like archery , prob an emperor title
something more common like marksmanship, swordmanship would be equivalent to the sort of prestige you see with an emperor because very likely the characters that hold titles in broad fields will tend to be emperors themselves
I mean most of the folks with a title are the heads of some of the most notorious organizations in the world with the exclusion of mihawk until recently.
