They underestimate how devastating Gamma Knife was. People almost die from some damage to their vitals, Whitebeard goes down on his knees from a heart attack. Yet having all of his organs ruptured is somehow reduced by 90% by stitching them together.

Pica couldn't do it.
So it's Buggy > Pica.
