It has nothing to do with the translators "Putting whatever they want", but rather them translating it in a way to convey the message to the target audience of what the author's intent was, in order to have it make sense. That's why Viz employs professionals who are capable of making those kind of distinctions so the message to the reader doesn't get lost in translation.

Well no shit. A hole through anyones stomach is enough to kill them. Which is what the message was meant to convey, and why it was written the way it was. It was not meant to be taken as Sanji was "scarred he was going to be killed" like people are making it out to be.
