Ungraded swords are so cuz their power is unquantified or unknown
Kikoku is unclassified but it has country destroying potential. That would make it worthy of Supreme grade by hype and yet it’s not cuz Oda likely didn’t want to put a hard cap on it + Law is special
I do not believe Gram or most of the 19 swords are classified as Saijo. Muramasa is the only one I can see being classified cuz it follows Oda naming convention for them. The others have a hidden meaning behind their proper name, there has been no Saijo that has this

Even in same grade there is difference and there are ungraded swords too that are very powerful.
Enma, Shusui and Wado have the same grade yet they are not equal.
