It seems pretty obvious to me, in this case his heart is beating just fine. He can restart his heart with g5, but he can’t magically imbue himself with stamina (at least not yet)

Him dying would make for a better story, but it’s very unlikely it will happen, not after Oda spent all this effort in making us feel sorry for him.
I do think that the “kill switch” Saturn ordered is going to play a role soon, but I expect Oda to maneuver around it somehow nevertheless.
the real question is not who, but why does Luffy need food in order to be able to regain the ability to move, while Kizaru, who he most assuredly neg diffed, is able to on his own accord?
Why would he have shanks go out of his way to say “I wasn’t off guard” if he was, in fact, off guard? Why would someone at shanks level risk a war to warn whitebeard about someone who got a cheap shot in while he was drunk?

In all of those cases, Luffy remained conscious longer than his opponent, and his opponent was cleanly ko’ed. Here Luffy is ko’ed and his opponent is still conscious. I’ll also say I’d classify each of those cases as an extreme diff.
