There is actual harmful male genital mutilation in some cultures (eg skinning of the member ,or cutting it in half ,often in the form of initiation rites) but we don't talk about these because it would expose the popular narrative as a biased lie. The popular narrative being that female genital mutilation is always worse than male, which further invisibilizes violence against men and boys.
Circumcision is sometimes done for medical reasons even. The other forms of mutilation (neither male nor female) are not.
