What you are describing is the state of Pakistan but this crisis is on-going for decades
Nuclear attacks are less viable now and this stands even more for a country which is in grip of a crisis.
I would list some reasons.
Mutually Assured Destruction (MAD): Any nuclear exchange between major powers would result in catastrophic consequences for both sides, deterring actual use.
Global Condemnation: The use of nuclear weapons would provoke intense global backlash, diplomatic isolation, and economic sanctions with no bail out plans pushing economy into long recession period.
Conventional weapons have become more advanced and effective as well.
The only time nuclear was used as a weapon of mass destruction was during ww2 on Japan, and we've seen how devasting it has been, even after more than half a decade, people living there are suspected of getting blood cancer, that's just a fraction of what pakistan and India hold, they have enough to destroy the whole world.
You're right about how it's very very unlikely, but theoretically possible, that will be the last resort for Pakistan, and in case they decided, it'll be the beggining of the end, WW3 will be inevitable
