Think about it. Dressrosa had some parallels to some scenes with the Gorosei and Imu.

One of Doflamingo officers was Sugar, who is an adult stuck in a child's body. Her ability turns people into toys, and makes people forget about you.

Imu has been forgotten, but uses the World as his playground. Why would they choose to do this if they had all this power? Why not openly rule the World? One reason could be, because his strength was taken from him, and he was left in a child like state. He can't openly rule the World, because he doesn't have the strength to do so.

What if Lili did this to him, which is why he seems to hold a great resentment towards her. We know the Ope Ope no mi sacrifices the user's life so the recipient can become immortal, and Lili disappeared after the Void Century. Perhaps Lili was the original user, and deliberately used it on Imu. It kind of locked him in a 900 year hell, where what he wanted was right in front of him, but he could never truly have it.

Would honestly be a great twist.

First, him ruling from behind the curtains has to do with the pact signed by 20 kingdoms - no one to sit on the throne. He's honouring it because if he reveal himself then those 18 nations will hold resentment.


Second, he hold grudge against Lily because of backstabbing. She not only refused to join them at MJ but scattered all the PGs across the world. He probably killed her because we know Lily never returned to Alabasta after that.
