I'm tired of explaining it.
You have to distinguish black rays that start from the impact point, which Oda uses quite randomly and without apparent coherence, with black trails that come from the parts/weapons imbued with AdvCoC.
That being said, the KoH is the flames + AdvCoC (black trails) and we didn't see that in Egg Head.
Also, if you don't believe me you can see that Zoro already had flames on his swords before creating the KoH, which appeared just when CoC woke up and the contrails appeared.
Edit: In addition to having the tagline En-Ou of course, which we haven't seen either.

Not every single lightning or discharge is ACoC. CoA lightning/discharge exist. Zoro's KoH/ACoC lightning/discharge looks different than his CoA lightning/discharge.
Oda is consistent with the way he draws Zoro's ACoC lightning. It has a very distinct look. Zoro's ACoC is drawn as large black lightning that wraps or flows around his swords. The lightning, if stretched out, is at least the length of his swords. Usually, longer.
ACoC/KoH:
KoH is flames/smoke combined with ACoC.
Zoro's CoA lightning/discharge is small and doesn't flow or wrap around his swords. It's not even a 1/3 the length of his swords

Hopefully the anime clears things up
Zoro was in KoH state against Lucci, he just never threw an attack so we never saw him use an actual KoH attack, if he used a named attack you would more than likely see lightning streaks in that state. Same deal against King, he wasn't constantly leaking haki, that state is a primer for KoH based attacks. Don't know what happened off panel for him to deactivate.
