Difference is Whitebeard's title was outdated by then, as we're shown multiple times. Marco said he got considerably weaker, for example. Dude literally had cancer. He was only the true WSM in his Prime, Primebeard is the only version of Whitebeard that could beat Mihawk (even then it's an extreme diff).
Mihawk's title has no reason to be outdated like Whitebeard's, he's in the prime of his life and hasn't had any displays of inferiority to other top tiers.
