I don't think he knows how it is made tbh. I think he got it from somewhere inherited like zoro did from ryuma.

Could be when she was murdered it passed onto him and he killed her murderer.

I think it makes sense, and as for why he hasn't used it yet i believe the blade behaves like enma and does massive destructive cuts akin to mihawk slice on the iceberg. This would be why he did not use it on the holy land, but it also gives him a gimmick so to speak. With his df he can sustain himself by drinking fodders around him giving himself energy sustaining his fighting and haki, and with the uncontrolled sword he is launching massive haki cuts at a simple swing draining his haki.
 
I agree with you but as I said, this is Oda we are talking about and I don't want to fall for his baits so I prefer to wait it out for in manga confirmation.
At the end it depends how Aramaki's blade is gonna look like the next time we see it. If it's black again, that's basically the third moment. Or are you gonna wait till a statement confirms it? It's also alright, better playing save rather than looking like a fool at the end.

Was Ryuma's Shusui also directly confirmed as a black blade the first time we saw him? I don't remember exactly when it was confirmed.
