The punch is just the setup. The real damage comes from the Haki being injected into the body causing internal injuries. It's exactly how Impact Dials, and Lucci's Rokuogan works. If Kaido can't be hurt externally, then internally it is.

The fact remains that you don't need to punch to cause massive damage with Penetration haki.

Again, a punch is not required for Penetration Haki to cause massive damage. Regardless of how it's setup up, the haki is inflicting the bulk of the damage.

If you play WoW, just imagine "ryou" being some kind of non-physical damage type ability, like shadow damage, for instance.
It might be just as strong as physical damage, but it ignores armor (in this case, Kaido's dragon scales). That's really just it. Just a gimmick.
